Patient Coordinator Full Time

Key Responsibilities:

• Coordinate and manage patient care activities, ensuring seamless communication between patients, healthcare providers, and administrative staff.
• Develop and implement patient care plans, ensuring they meet both medical and personal needs. Monitor and evaluate patient progress , Communicate to the consultant for adjusting care plans as necessary to achieve optimal outcomes.
• Serve as a liaison between patients and healthcare providers, addressing any concerns and ensuring patient satisfaction.
• Oversee scheduling, appointment coordination, and follow-up care.
• Maintain accurate patient records and ensure compliance with healthcare regulations.
• Collaborate with healthcare professionals to improve patient care processes and outcomes.
